First Visit?
Visiting your dentist or periodontist doesn’t have to be a headache. With the help of Dr. Urbanski’s caring staff, your treatment can be smooth and comfortable. Here’s what to expect during your first visit:
- Prior to your first appointment, we’ll collect some basic information about your health and your dental care history. We've posted our New Patient Form to this website so that you can review it and/or complete it prior to your first appointment. To view our Patient Forms section, please click here.
- There are a few things you should remember to bring along for your first appointment. If you have dental insurance, remember to bring a signed form with your portion completed (if applicable). You should also bring your dental and medical insurance cards with you.
- If you normally take antibiotics prior to dental appointments, please let us know, so we can prepare a prescription. It is very important to take them for all appointments unless otherwise instructed.
- When you arrive for your appointment, we’ll start by reviewing your health questionnaire. Then you will receive a thorough periodontal examination. Depending on your needs, we will take X-rays and prepare study models to help us establish a diagnosis and a treatment plan. Feel free to ask questions at any time during your examination or treatment.
- You may meet with our financial coordinator, Sandy, to discuss insurance coverage and financing options.
6. While we understand sometimes it may be necessary to change an appointment, it is our office policy to require 48 hours notice of a cancellation. Please be advised a fee can be administered for non-compliance with this policy.
